How the Glasgow Coma Scale (GCS) Calculator works
The Glasgow Coma Scale adds the best eye (1-4), verbal (1-5) and motor (1-6) responses, giving 3 to 15. Recording the three parts separately (for example E3 V4 M5) is more informative than the total.
A score of 8 or less usually indicates a severe brain injury. This tool supports assessment by trained staff; it is not for self-diagnosis.
The formula
Worked example

| Eye opening | 4: Spontaneous |
|---|---|
| Verbal response | 5: Oriented |
| Motor response | 6: Obeys commands |

Result:
| GCS score | 15 |
|---|---|
| Written as | E4 V5 M6 |
| Head injury severity | Mild (13 to 15) |
Frequently asked questions
What if the patient is intubated?
The verbal score cannot be tested; it is recorded as ‘T’ (for example E3 VT M5) and the total is not comparable.
